In addition to the form above, for private high school applications (Christian Brothers, Jesuit, St. Francis, Cristo Rey, etc.) please include Ms. Payne's email address on their respective on-line application when requesting academic records.  Transcripts will be sent directly to the school.  Email:  Michele-Payne@scusd.edu
