Athletic Program
Miwok Middle School Athletic Program provides equal access for all students. It guarantees opportunities for maximum participation, through competition in athletic programs which promote team work, sportsmanship, self-esteem, school spirit, and positive social skills.
Our sports teams are very competitive. Through the years our teams have won several city championships in all the sports.
Sports Participation Clearance: We will be using the Home Campus online system for sports participation paperwork for your child to be cleared to participate for the 25-26 school year. You will need to create a profile online and complete the required paperwork. See details at the bottom of this page. Once on file, these forms will be sufficient for any and all school sports for the 2025-2026 school year.

Sports Tryout Physicals
If you are in need of a sports physical before you try-out for a Miwok Middle Sport Sockolov & Sockolov offer pre-participation sports physicals for athletes. They’re offered Monday through Thursday starting at 4:00 pm. Bring a SCUSD Medical Evaluation form (download sports packet) and $30 cash. The address is 1 Scripps Drive Suite 202 Sacramento, CA 95825 and can be contacted at (916) 927-1114.

- To be eligible for athletic participation, each student must complete and submit the online application accessible at homecampus.com. School administration will review the application and notify the student and parent of clearance to participate in athletics. Instructions for completion and submission of home campus application is at this link.
- Athletes must have a yearly physical from a qualified physician who completes, signs, and stamps the SCUSD Sports Physical Form. The signed and stamped form must be uploaded into the home campus account in order to be considered for athletic clearance.
- Fill out the ASES Program Application.
